I guess you could count the soul carin as another world you can travel too, but it's much much smaller than the shivering isles, and only adds a few more new creatures.

Down the street, there's horses is someone's backyard. We're in a small town, lol. Almost everyone has horses, cows, or chicken in their yards in this area.
Nothing screams 'small town' louder than farm animals in the backyard.
