As a Brit in the US of A, I'm tempted to reply that it's only the Americans in the UK that use it. (And leave you to determine whether I meant heroin, or the term "smack"). 
The history is that Britain ran the majority of the Opium trade back when it had the world's largest navy. So the raw opium was the original drug (and tinctures such as laudanum) Sherlock Holmes was hooked on Opium. Then the Germans refined morphine from it in the 19th century, and heroin was derived from that in the UK. The medical school where that happened is part of the Imperial College of Science, Technology and Medicine, where I got my degree. Of course, we'd call it diamorphine, because we're scientists.
Heroin was actually Bayer's brand name for diamorphine. Yes, the Aspirin people in Leverkuessen, Germany. Street names are various, and don't appear to have much regional preference.
Cockney Rhyming slang will derive from the street names, rather than "heroin", so "Uncle Mac" is one name for it.
