Now that you mentioned the follower...does it have to do with Nick Valentine? The final location, where his quest takes place, is called Andrew's Station, though I don't recall this particular stage, I know there's some underground bar, so maybe it's the same place.

Close enough for me! The bar in particular is called the Southie Speakeasy, and is at the end of Andrew Station.

Honestly, I think that the location is easy to figure out, but to drop out one hint - Goodsprings isn't the only "springs" in the wasteland.

Bitter Springs is unfortunately wrong.
It's the "third Springs", quite close to Goodsprings actually.
Also, the raider in combat armor is unique and carries unique knuckles. Once you kill the raiders, cazadores take over the place.

You can see that mountain from most of Skyrim, and she may well have been outside Windhelm. I could only determine North-eastish and looked for a high enough place you could see that far. Shearpoint is more North and Windhelm more to the East.

No. Skingrad is not yet in ESO, so you can't get near it, and Fat Ramp isn't on the map. This location is below ground.

Alright let's try to get a crack on this. Lena said Underpall Cave, and ghastley said Lena is "almost right". So I am using one of my lifelines.

Man, nothing is near Underpall. Chorrol is nearest town I guess, but Chorrol is also wrong. So is Bruma. Grrraawal... Yeesh I am stumped.
Maybe Lipsand Tarn? Maybe Lipsand looks way different in Elder Scrolls Online's 2E world?

Well, actually my first hunch was Fingerbowl Cave. Let's try that. I have no idea which locations from TES4 are available in ESO, so I'm just going through TES4.

Does it look very different from TES4 or is it fairly similar? Don't give the details, just the degree of similarity, please.

The outside is very similar to TES 4. The interior (where I took the screenshot) is quite different. Both have water, but in ESO you can wade everywhere except the back exit. Oblivion had bandits, where ESO has goblins.

Well, I'm stumped. Since the interior is different in TES4, I won't be able to recognise it, unless you also post a picture of the exterior. Underpall had undead, this place has bandits, and yet Underpall was close, although not geographically... umm... Fort Cedrian? A shot in the dark. We nearly drowned there, so it definitely has water...

I was about to start thinking "fort" instead of town or city. Never would have guessed Undertow though, my next guess would have been whatever fort is nearest to Underpall.

My thinking is to blame it on mass and scale. ESO is so much bigger than Oblivion and Skyrim combined that remembering locations is practically impossible for me. By the time Buffy gets back to visiting someplace she's already been, she's forgotten it and it feels new. Currently, ESO features about 35 'zones'. On average, each zone has as much content/locations as either Oblivion or Skyrim.
Undertow Cavern is one of 6 types of dungeons, called delves. On average, each of those 35 'zones' has 6 delves. 6x35 is. . . alot.
